Dr Saunders deliberately created the hospice as an independent charity outside the NHS so that she could develop care beyond that required by a national health service.
There has been lots of debate as to whether this was the right decision, and if end of life care would be better now, had she chosen to re-integrate hospice care into the NHS once it was fully established.
